Dear Patient:
After four years in practice with Cardiothoracic and Vascular Surgeons, my last day with the group will be September 30, 2025. I will remain in Austin, transitioning my practice to Texas Oncology starting October 1, 2025. It has been richly rewarding to know you and to take care of your medical needs. I would be honored to be able to provide care to you going forward here in Austin. If you wish to transition your care to Texas Oncology, please call their office at 512-421-4250 and ask to speak to the New Patient Coordinator.
If you prefer, you can also continue to receive excellent care through one of my colleagues at Cardiothoracic and Vascular Surgeons. For more information about my colleagues, please visit www.ctvstexas.com.
Your medical records are confidential. A copy can be transferred to another surgeon or released to you (or another person you designate) only through your permission. My office staff will be available to help you with the transfer of your medical records and other administrative matters. If you wish to transfer your records to a surgeon other than me (or outside of CTVS), please call our office at (512) 459-8753 with the request, name and date of birth, as well as the physician/office to whom the records should be sent.
It has been an honor and pleasure to work at Cardiothoracic and Vascular Surgeons and to serve as your surgeon.
With Warmest Regards,
Matthew A. Gaudet, M.D.
Thoracic Surgeon
