Dear Patient,
After nearly ten years in practice with Cardiothoracic and Vascular Surgeons, my last day with the group will be October 31, 2025. I am relocating to San Antonio and joining the Methodist Hospitals to take over as chief of cardiothoracic surgery for their network. It is a unique opportunity that will allow me to utilize my skills to help shape and grow cardiothoracic surgical care in their community. However, I do so fully embracing and appreciating my experiences here, especially with patients like you. I am deeply honored that you have given me your trust and the opportunity to care for you. It has been richly rewarding to know you and to have been part of your medical journey.
As my patient, you can continue to receive excellent care through one of my colleagues at Cardiothoracic and Vascular Surgeons. For more information about my colleagues, please visit www.ctvstexas.com.
Your medical records are confidential and a copy can be transferred to another surgeon or released to you or another person you designate only through your permission. My office staff will be available to help you with the transfer of your medical records and other administrative matters. If you wish to transfer your records to a surgeon outside of CTVS, please call our office at 512-459-8753 with the request, name and date of birth, as well as the physician/office to whom the records should be sent.
It has been an honor and pleasure to work at Cardiothoracic and Vascular Surgeons and to serve as your surgeon. Nothing could have been more gratifying for me. I wish you good health and the best that life has to offer.
Best regards,
Jonathan Yang, MD
Board Certified Cardiothoracic Surgeon
